I have no money and a court order for visitation that is not being up held

I had my will, living will, and power of attorney prepared in the state of Arizona in 2015. I moved to Utah August 2017. Can I prepare amendment or codicil to these documents to indicate that I no longer live in Arizona? Do I need witnesses sign the updated form ? What steps do I need to do to make my documents legal? My attorney who prepared the documents has been notified three times (over a month ago) and I have not heard from him or his office with an answer to make my documents legal. I would appreciate any help in this matter so I can get matter taken care of. Thank you in advance for your help.

I sent a request about how to make my will legal in Utah after I had the documents (Will, Power of Attorney, and Living Will) prepared by an attorney in Arizona. I was living in Arizona at the time of the preparation of the documents. I would like to get this taken care of as soon as possible. Thank you in advance for your help. It has been a week since I submitted my inquiry on making my will legal in the state of Utah. I have no plans to ever move back to Arizona.
